Case Management(Community Psychiatric Supportive Treatment) is available to improve the quality of life for persons with severe and persistent mental illness. The program provides individualized service that ranges from assisting clients in meeting their basic needs to encouraging growth and independence.
Employment Services: Century Health has a vocational specialist available to work in conjunction with case managed clients to assist in obtaining employment. We use the supported employment model which is an evidence-based practice that monitors the client through four stages to attain employment.
Assertive Community Treatment Team (ACT): This is a multidisciplinary treatment program that provides intensive support and comprehensive treatment to the most severely and persistently mentally ill adults in our system. This is a specialized service for clients that do not respond to traditional case management and have demonstrated a need for 24 hour access to treatment providers.
Integrated Dual Disorder Treatment: Century Health uses this evidence based practice that utilizes a multidisciplinary team to provide services for clients with substance abuse and severe and persistent mental illness. This is a unique, integrated approach that promotes recovery through four stages of interaction with consumers and caregivers.
